List of required documents for CM License:
- Application in prescribed format and letter head pad of the company
- Copy of the Trade License
- Copy of the Trademark registration
- TIN Certificate/Income tax return proof
- Copy of the Premises License – for food items
- The label of the product must reflect the following information in Bengali Language:
- Name of the product and Brand name
- Grade/Type/Size/Voltage (if required)
- Weight/Quantity
- Code/Batch number
- Composition of the product
- Date of Manufacturing/Packaging
- Date of Expiry
- Name and address of the Manufacturer/Packaging company with address

Test Fees: Depends on the specific product type/chemical profile. Varies product-to-product (as per the Lab schedule of BSTI)
Marking Fees (License fees):
While Local manufacturers pay a marking fee based on the Ex-Factory price (0.07% for Fruit items and 0.10% for other products), imported goods are assessed uniformly at 0.10% of the total CIF value as on the commercial invoice and the Bill of lading.
